At Attune Menopause Care, we're committed to providing you with excellent care while maintaining transparency about the financial aspects of your healthcare. This policy outlines what you can expect from us and what we ask of you regarding payment and billing.

Payment Options

We accept both insurance and self-pay patients. Your financial responsibilities will depend on which payment method you choose.

For Patients Using Insurance

Your health insurance policy is a contract between you and your insurance company. While we're happy to work with your insurance as a courtesy, it's important to understand your coverage and financial responsibilities.

Insurance Verification

We will verify your insurance benefits as a courtesy to help you understand your expected costs. However, please know that benefit verification is not a guarantee of payment. Insurance companies may process claims differently than the benefits quoted to us, and the final determination of coverage is made by your insurance plan.

We strongly encourage you to also contact your insurance company directly to confirm:

  • Whether our services are covered under your plan

  • Your deductible amount and whether it has been met

  • Your co-pay or co-insurance amounts

  • Whether any prior authorization is required

Your Responsibilities

To ensure smooth processing of your insurance claims, you are responsible for:

  • Presenting your current insurance card at each visit

  • Providing a photo ID (driver's license preferred) for identity verification

  • Notifying us immediately of any insurance changes and providing a new insurance card (we need copies of both front and back)

  • Obtaining any required referrals or prior authorizations from your insurance company before your visit

For Self-Pay Patients

If you choose not to use insurance or do not have insurance coverage, you are considered a self-pay patient. Self-pay patients are responsible for payment in full at the time of service.

Self-Pay Rates

Self-pay patients will be charged our standard self-pay rates for services. We're happy to provide you with a Good Faith Estimate of costs before your visit so you know what to expect. Simply call our office at (872) 253-4891 to request an estimate.

Financial Assistance

If the cost of care is a barrier for you, please let us know. We may be able to work with you on payment arrangements or offer a discount based on your individual circumstances. We're committed to making quality menopause care accessible.

Payment for Services

Payment at Time of Service

For Insurance Patients: Co-payments and any known patient responsibility amounts are due at the time of service.

For Self-Pay Patients: Payment in full is due at the time of service.

We accept cash, check, Visa, MasterCard, American Express, and Discover.

Credit Card on File

We require a credit card on file for all patients. This allows us to process co-pays, co-insurance, deductibles, and any remaining balances efficiently.

For insurance patients, once your insurance company has processed your claim and we've received their payment, we will charge your card on file for any patient responsibility amount (such as deductibles, co-insurance, or non-covered services). You will receive a statement showing the insurance payment and your portion before your card is charged.

Patient Financial Responsibility

You are financially responsible for all charges incurred during your care with us, including:

  • Co-payments required by your insurance plan

  • Deductible amounts

  • Co-insurance (your percentage of the cost after insurance pays their portion)

  • Services not covered by your insurance plan

  • Services provided before your insurance effective date or after your coverage ends

This responsibility applies whether or not your insurance company pays the claim, and regardless of any contract status between Attune Menopause Care and your insurance carrier.

Non-Covered Services

Some services we provide may not be covered by your insurance plan or may not be considered medically necessary by your insurance company, even though we believe they are important for your care. You remain responsible for payment of these services. Common examples may include certain lab tests, procedures, or treatments that fall outside standard coverage.

How Billing Works

Filing Insurance Claims

As a courtesy, we will file claims to your insurance company for all covered services. We will submit complete and accurate claims in a timely manner to help ensure proper processing.

After Insurance Processes Your Claim

Once your insurance company processes your claim and sends payment, we will apply that payment to your account along with any contractual adjustments. If there is a remaining balance (such as your deductible, co-insurance, or non-covered services), you will receive a statement showing the amount due.

Payment Terms

All patient responsibility balances are due within 30 days of the statement date. If you receive a statement from us, please remit payment promptly.

If you have questions about your statement or are experiencing financial difficulty, please contact our billing team at (872) 253-4891. We're here to help clarify any confusion and work with you on payment arrangements when needed.

Assignment of Benefits

By signing this policy, you authorize and direct your insurance company to pay Attune Menopause Care directly for services rendered. You also authorize us to release any medical information necessary to process your insurance claims and receive payment.

If your insurance company sends payment directly to you for services we provided, you agree to endorse and forward that payment to us immediately.

Cancellation Policy

We maintain a 48-business-hour cancellation policy. Please see our separate Cancellation Policy document for complete details. In brief:

  • Appointments must be cancelled at least 48 business hours in advance

  • Late cancellations or no-shows are subject to a $50 fee

  • After three no-show occurrences within 12 months, we may need to discontinue our patient relationship with you

Updates to This Policy

We reserve the right to update this financial policy as needed. Any changes will be posted on our website and provided to you at your next visit. Updated policies apply to all services provided after the effective date of the change.

Patient Acknowledgment

I have read and understand Attune Menopause Care's Financial Policy. I understand that I am financially responsible for all charges incurred for services provided to me.

☐ Insurance: I authorize Attune Menopause Care to file claims to my insurance company on my behalf and to release any information necessary to process those claims. I assign my insurance benefits to be paid directly to Attune Menopause Care for services rendered. I understand that I am responsible for all charges whether or not paid by my insurance company, including co-pays, deductibles, co-insurance, and non-covered services. I understand that my credit card on file will be charged for any patient responsibility amounts after my insurance has processed the claim.
