Schedule Your Follow-Up Visit
Ready to book your next appointment? You can schedule directly online for either in-person or telehealth visits.

Need to Reschedule or Cancel?
To reschedule:
Message us through the Charm portal or Spruce app to let us know
Go to the scheduling portal and select your new date and time

Please give us at least 48 hours' notice. Late cancellations or no-shows may incur a $50 fee, with exceptions for emergencies or illness.

Access Your Health Records
Attune uses Charm Electronic Health Records for your patient portal. Through Charm, you can:
Complete pre-visit questionnaires
Review visit summaries and care plans
Access lab and imaging orders and results
View and download handouts and visit notes from your visits
Request prescription refills
Upload lab results and medical records from care with other providers
Update insurance information

Message Your Provider Securely
Have a quick question between visits? You can reach us through secure messaging via either the Charm patient portal or the Spruce Health app - whichever you prefer.
A note on privacy: To protect your health information, please don't send medical questions or details via regular text or email. If you reach out through these channels, we'll ask you to use Charm or Spruce instead.
For Urgent or Emergency Concerns:
Emergencies: If you're experiencing a medical or mental health emergency, call 911 or go to the nearest emergency room immediately. Do not use secure messaging for emergencies.
Urgent (but not emergency) concerns: Call our office at (872) 253-4891 during business hours (M-F 8:30am-4:30pm). For urgent medical concerns that can't wait, contact your primary care provider or visit an urgent care center.
Secure messaging is not monitored in real-time and should only be used for non-urgent matters.
Please reach out anytime something feels off—if your concern requires treatment adjustments or detailed review, we'll schedule a follow-up to give it our full attention.
What to use secure messaging for:
Brief administrative questions (appointment scheduling, paperwork)
Quick clarifications about instructions from your last visit
Pharmacy issues (your pharmacy is out of stock, need prescription sent elsewhere)
Prescription refill requests to get you through to your scheduled follow-up
Letting us know you've completed labs or picked up a prescription
Non-urgent updates (e.g., "The new medication is working great!")
When you need to reach out AND schedule a follow-up visit:

How to Access Secure Messaging
Charm Portal: Log in through your patient portal
Spruce App: If you haven't been invited to Spruce yet, text (872) 253-4891 requesting a Spruce invite.

Can't access Spruce? Check the Spruce Help Center or call our office at (872) 253-4891.
Response time: We typically respond to brief messages within 1-2 business days. For clinical concerns, we'll direct you to schedule a visit.
Prescription Refills
For routine refills: Request through your Charm patient portal or Spruce messaging. Please allow 2-3 business days for processing.
Running low? Request refills at least one week before you run out to ensure you don't have a gap in treatment.
Need a refill urgently? Call your pharmacy and ask them to contact our office directly.
Regular Visit Requirements: To ensure safe prescribing, testosterone therapy requires regular monitoring with lab work and follow-up visits. Your provider will review your specific schedule with you. It is your responsibility to keep track of when you need to complete lab work and schedule follow-ups in accordance with this plan.
Saving on Prescription Costs
Prescription costs can vary widely depending on your insurance coverage. We encourage you to compare prices and explore discount options, especially if you're facing high out-of-pocket costs.
Discount Programs for Generic Medications:
GoodRx - www.goodrx.com
Compare prices at pharmacies near you and download discount coupons. Present the GoodRx coupon to your pharmacist before paying. Note: GoodRx discounts cannot be combined with insurance, so compare both options to find your lowest cost.
Cost Plus Drugs - costplusdrugs.com
Mark Cuban's mail-order pharmacy offers transparent pricing on many generic medications, often at very low cost. This option doesn't work through insurance but may be cheaper than your insurance co-pay. Particularly helpful for those without insurance. They carry vaginal estrogen cream at significantly lower prices than many pharmacies.
To use Cost Plus Drugs: Create an account at costplusdrugs.com using the same email address you use for your Attune patient portal. When we send your prescription to Cost Plus, they'll match it to your account using your email address.
Manufacturer Savings Programs for Brand-Name Medications:
If you've been prescribed a brand-name medication, the manufacturer often offers savings programs or co-pay cards that can significantly reduce your cost:

Important: You're responsible for comparing prices and determining which option works best for you. We don't know your specific insurance co-pays or coverage, so we encourage you to check multiple sources before filling your prescription.
Lab Work & Test Results
Viewing results: Lab and imaging results are posted to your Charm patient portal as soon as we receive them. You'll get a notification when new results are available.
Understanding results: Your provider will reach out to discuss any results that need attention or follow-up. If you have questions about results you've viewed, send a message through Spruce or the Charm portal, or schedule a follow-up visit.
Ordering labs: If your provider has ordered labs, you'll find the orders in your Charm portal along with information about which lab to use and any preparation needed.
Billing & Insurance Questions
Insurance Card Updates
If your insurance information has changed, please upload your new card to the patient portal:
Log into the Charm patient portal
Navigate to the Documents section from the main menu
Select "Upload"
Choose the photo or PDF from your device (or take a photo with your mobile device)
Click "Upload" to complete
Don't forget to upload both the front and back of your card
