

New Clients
New clients are welcome to directly schedule through Google Appointments using the Book Now feature.

Once you have requested your desired appointment time, we will be in touch to confirm your visit and provide you with the consent and intake forms needed to establish your care with us. We ask that you complete this paperwork as soon as possible. It is easiest to complete the forms on a laptop or desktop computer. Paperwork must be submitted at least 48 hours prior to your visit.

In-person appointments are offered at Urban Wellness Chicago. The space is on the 2nd floor of a building that does not have an elevator. Due to this limitation, we are not able to offer in-person appointments to those with a mobility limitation that prevents them from being able to navigate stairs.

Telehealth services are available to clients in Illinois at the time of the video appointment.

Payments and Insurance
Attune Menopause Care PLLC is in-network with BlueCross BlueShield of Illinois PPO plans, UnitedHealthcare, and Aetna.
​
We are in process contracting with other insurance carriers. Please reach out and we can check if we are in-network with your insurance plan.

For other private insurance or those who are uninsured, we accept self-pay at this time and will provide a superbill that you may use for FSA reimbursement or submission to insurance.

Self-pay pricing:
New patient (60 minute appointment) $250
Returning patient (30/60 minute appointment) $125/$200

We regret that we cannot see clients with Medicare or Medicaid coverage, even for a self-pay visits at this time.
