This is on the second floor of a building without an elevator. We are unable to see those with mobility limitations who cannot use stairs at this space.
Pre-visit paperwork & late cancellation/no-show fee
After booking, you’ll receive a Charm EHR invitation with required forms. Forms must be completed at least 48 hours before your visit.
We require 48 hours’ notice to cancel; late cancellations or no-shows are subject to a $50 fee.
